Use this form to report a concern involving township roads, drainage, signs, facilities, cemeteries, services, zoning or another township matter. For emergencies or immediate danger, call 911.

Milford Township does not accept anonymous complaints. All required fields, the certification and your signature must be completed before the form can be submitted. You can also return a printed form to Milford Township Hall, 4651 Wall Street, Centerburg, Ohio 43011.
